| I-20 Request Form to Replace Lost, Stolen, or Damaged I-20 |
This form is used when a student needs a replacement I-20. |
| I-20 Request Form for Extension |
This form is used by students who require additional time to complete academic program requirements and are requesting an extended I-20. |
| Academic Advisor Recommendation Form for I-20 Extension |
This form is to be filled out by an academic advisor who is recommending that a student be issued an extension I-20. |
| I-20 Request Form for Use by Dependents |
This form is used to request new I-20s for dependents who wish to enter the US or change status to F-2 status. |
| I-20 Request form for Continuing Students |
This form is used by students who are continuing U-M students but who need an initial, transfer back, or reinstatement I-20. Do not use this form if you are a new U-M student; contact your admissions office for your initial I-20. |
| Financial Resources Statement |
This form must accompany certain I-20 request forms. It is used to certify sufficient funding for the issuance of a new I-20. |
| Financial Affidavit |
Each individual contributing financially to the named student's education must complete the Affidavit of Financial Resources. |
| I-20 Travel Signature Drop-Off Form |
This form is needed to request official I-20 authorization to reenter from a trip outside the U.S. |
| Standard Letter Request Form |
This request form is used to ask for one of three standard letters: enrollment letter verifying your full time enrollment at U-M; estimated expenses letter indicating the amount of funding necessary to continue your studies at U-M; or guest student letter indicating your eligibility to attend another school during your summer vacation. |
| F-1 Student Reduced Course Load (RCL) Request Form |
This form is used to request approval for Reduced Course Load (RCL) enrollment from the U-M International Center. |
| Full Time Equivalency for Graduate Students | This form is used to request a certificate of full time equivalency from the U-M International Center. |
| F-1 Severe Economic Hardship Request Form |
This form is used to request recommendation for severe economic hardship employment authorization. |
| F-1 Transfer Out Form & Checklist |
This form is used to notify U-M of your intent to transfer your F-1 immigration status to another school. The U-M International Center uses the information to update your SEVIS record with the name of the school to which you will transfer and your transfer release date. The checklist guides you through the process of officially transferring from U-M to another educational institution in the U.S. |
| F-1 International Student Departure Form |
This form is used to notify the U-M International Center that you are leaving U-M or the U.S. temporarily or permanently. The U-M International Center uses the information to adjust or end your SEVIS record. |
| I-20 Request for LSA Undergraduate Cross-Campus Transfer |
This form is used by Undergraduate LSA students who have received approval from the LSA Advising Center to transfer from another UM school to LSA and need an I-20 to reflect the new academic program. This I-20 must be obtained before beginning study in LSA. |
| I-20 Request for LSA Undergraduate Reactivation |
This form is used by LSA students who took an academic break for two or more years and are now returning to U-M LSA. Students using this I-20 request form must have already received confirmation of the reactivation from LSA Academic Advising. |
| F-1 Student Update Personal Information on I-20 Form |
This form is used by F-1 students who have a change or correction in their personal information (name, citizenship, birthdate) and wish to be issued an updated I-20 reflecting this change. |
| Remove Dependent |
This form is used by F-1 students who wish to remove their F-2 dependents from their I-20. |
| Update Financial Information |
This form is used by F-1 students who have a significant change in their funding situation and wish to be issued an updated I-20 reflecting this change. |
| Social Security Letter for U-M F-1 Students |
This letter is used by F-1 students who have secured on-campus employment and need to verify employment and student status in order to apply for a social security number. Please read Getting a Social Security Number and Card |

This form is used by professors, research scholars, short-term scholar, or specialists. It is used to request a new DS-2019 if you need to replace a lost form, add dependents, or edit financial information. |

J-1 Scholars receive this signed Social Security letter at check-in. If you have not yet attended the mandatory check-in, please wait and do not download this letter now. If you have attended check-in and subsequently lost this letter, print this on-line letter and bring it to the International Center for a signature. If you are unable to pick-up documents at the U-M International Center, there are two options available to you. Select one of the methods to the left.
- Express Mail (eShip Global) is the preferred method and is a discounted online service. You will need a UMID to create an account and you will be charged for this service. After you create your account, you must fill in the “Reference/Comment” field in your shipping form with the documents you are requesting to be shipped. Failure to fill in your requested documents will make it impossible to ship.
- Regular First Class U.S. Mail is not recommended, is for U.S. addresses only, and there is no charge.
