| SEVIS DS-2019 Request For Use by J-1 Scholars |
This form is used by professors, research scholars, short-term scholar, or specialists. It is used to request a new DS-2019 if you need to replace a lost form, add dependents, or edit financial information. |
| SEVIS DS-2019 Travel Signature Drop-Off Form |
This form is needed to request official DS-2019 authorization to reenter from a trip outside the U.S. |
| J-1 Transfer Out Form |
This form is used to notify U-M of your intent to transfer your J-1 immigration status to another school. The U-M International Center uses the information to update your SEVIS record with the name of the school to which you will transfer and your transfer release date. |
| J-1 Visiting Scholar Early Departure Form |
This form is used to notify the U-M International Center that you are leaving U-M or the U.S. The U-M International Center uses the information to adjust or end your SEVIS record. |
| J-1 Exchange Visitor Information Form |
The exchange visitor information form is for newly invited exchange visitors to provide information to their host departments. Do not submit this directly to the International Center; return it to your inviting academic department. |
| Social Security Letter for U-M J-1 Scholars |
J-1 Scholars receive this signed Social Security letter at check-in. If you have not yet attended the mandatory check-in, please wait and do not download this letter now. If you have attended check-in and subsequently lost this letter, print this on-line letter and bring it to the International Center for a signature. Please read Getting a Social Security Number and Card. |

If you are unable to pick-up documents at the U-M International Center, please fill out online & print this form to provide us with shipping information. We will arrange for shipment of the documents via UPS. The cost of shipping is your responsibility, and we have found that the most secure way to pay is with a UPS account number. If you do not have an account, you can easily create one for yourself. Instructions are on the UPS Shipping Information Sheet form.
